MLTSS Consumer Flyer Updated

Pennsylvania is making major changes to the delivery of long term care. The changes will impact all those who are eligible for both Medicare and Medicaid (dual-eligible) as well as those who are considered Nursing Facility Clinically Eligible (NFCE) and are receiving services in their homes or in a nursing facility. CARIE updated the MLTSS consumer fact sheet created to help educate older adults about some of the changes and how they can participate in the planning process.  The changes include adding the new name Community HealthChoices, identifying the exclusion of LIFE participants from the plan, and updating DHS’ new website address.  Please continue to help us educate consumers and their caregivers by distributing this flyer.
